HSA/FSA Instructions
Overview
You can use your HSA/FSA funds for public classes or private sessions with Rowena Mittal Yoga by following these simple and free steps below.
1. Obtain a letter of medical necessity (LMN) from your healthcare provider

A healthcare provider who can provide you with a letter of medical necessity can be your primary care physician (PCP) or a specialist. Send your provider a message using their online app or calling their office and asking how to submit the request to your provider by other means. Some providers may require you to make an appointment to discuss your condition and ask for this LMN if you haven't seen them in a while.

To make sure your LMN isn't missing any information, you can call your HSA/FSA provider to learn exactly what they need to see in an LMN ahead of making your request to your PCP. Here is a template request to your healthcare provider that you can use and personlize to your specific condition(s):

I hope this letter finds you well. I am writing to formally request a Letter of Medical Necessity (LMN) to support my participation in therapeutic yoga classes and private sessions with Rowena Mittal Yoga. Given my medical history and current health conditions, I believe that a structured and personalized yoga practice will significantly contribute to my overall well-being and symptom management.
I have been managing [list medical conditions such as chronic pain, arthritis, anxiety, depression, hypertension, back pain, fibromyalgia, postural imbalances, etc.], and I am seeking yoga as an adjunct therapy to alleviate symptoms and improve my quality of life. Research has shown that yoga can provide the following benefits for these conditions:
- Chronic Pain & Arthritis: Improves joint mobility, reduces inflammation, and enhances pain tolerance.
- Anxiety & Depression: Promotes relaxation, reduces stress hormones, and fosters a sense of well-being.
- Hypertension & Cardiovascular Health: Lowers blood pressure, improves circulation, and enhances heart rate variability.

- Fibromyalgia & Fatigue Syndromes: Reduces muscle tension, improves sleep quality, and enhances energy levels.
Rowena Mittal, MS, PhD, CYT, is a highly qualified yoga instructor with expertise in both biomedical scientific research and therapeutic yoga applications. She provides tailored yoga instruction that aligns with individual needs, ensuring safe and effective practice. The sessions focus on movement modifications that foster strength and mobility, breathwork, myofascial release and relaxation techniques to support my specific health concerns.
An LMN from you would allow me to submit a claim for potential reimbursement through my health insurance or Flexible Spending Account (FSA)/Health Savings Account (HSA). If you require any additional information regarding Rowena Mittal Yoga or my proposed yoga regimen, I am happy to provide further details.

If you don't hear back from your PCP within a reasonable timeframe for your needs to start a therapeutic yoga practice, call their office to followup on status.
2. Submit the LMN to your HSA/FSA provider or insurance company.
3. Use your HSA/FSA card to book classes or private sessions with Rowena Mittal Yoga.
4. Submit the receipt to your HSA/FSA provider as needed. Include a copy of the LMN as needed.
